Life as a physician is sometimes like a runaway trailer

It was a hot day in Northern California and my clothes were covered in red summer dust. At the age of 9, I couldn’t step onto our property without locusts pouncing from the dry wheatgrass. Colorful ladybugs were also out and about in large numbers. I looked everywhere for a container to store my prized bugs, but my efforts were unsuccessful. I went to Mr. Maryfield’s trailer, which was parked on a hill on the property line we shared. I passed the trailer. The temptation to peek inside was strong, despite my father’s multiple warnings not to go near it. My head spun around, searching for some sort of insect prison. Trailer in sight, I hesitated. “Don’t do it,” I could hear my father say. “It is too dangerous.” But my desire to find a bug container was strong.
Despite Dad’s warning, I climbed onto the front of the trailer’s iron towing eye and peered over the side wall. There it was! A beautiful blue coffee tin from Maxwell House in the front corner just below me. But I was a nervous cat and didn’t jump off the trailer once but three times to build up my courage. I looked around like a shoplifter contemplating a candy bar robbery.
The trailer was heavy and wouldn’t budge an inch when I mounted it for the third time. It had twin wheels and metal bars supporting a wooden case. When I was sure no one was looking, I leaned over the side wall until my body was upside down.
I felt my feet were up in the air now and I remember feeling vulnerable. I reached for the can and barely managed to grab it by the lip. My heart was pounding, but stopped short as I felt the trailer begin to move.
I was instantly terrified, but not out of fear for my safety. What I feared most was my father’s wrath. I managed to get back on the hitch with the precious can in hand. But the trailer picked up speed and with my back to the road I was now part of it.
I knew I had to act to prevent a catastrophe. The trailer picked up speed. A cloud of red dust surrounded me and I couldn’t see the driveway below. At that moment, I jumped off the front end and landed on my feet while holding on to the iron tow bar. I pushed against the tow mount as if my fourth grade strength was slowing its forward motion. It shouldn’t be as the powerful trailer pushed me.
I surfed backwards on the soles of my now burning high tops. With my body stretched out and my shoes digging into the driveway, thick dust covered my mouth.
Surely I was in the middle of a cattle stampede trying to bulldog a giant ox. The trailer took me halfway down the driveway until my legs collapsed and my grip on the hitch went loose. I was sucked under the trailer and then thrown sideways, leaving marks where the tires had crossed my shins. My father was there and saw the whole incident. He picked me up off the ground and hugged me. There would be no retribution, just a celebration of life. The trailer spun and crashed into a tree at the end of the driveway, effectively avoiding disaster.
When you first discover your desire for science and medicine, it’s all consuming. If you decide to pursue a career in medicine, you better be prepared. The path to the prize can be tiring with many obstacles.
You will have many questions. What is necessary? How does someone with average grades get admitted to medical school? Those around you will be happy to tell you how difficult it is and how much sacrifice you have to make.
What about your wife and child? How will you support them and still study? But don’t be put off. you like challenges Despite what others tell you, you reach for the blue coffee can. And then something amazing happens. You will receive a notification of admission.
Congratulations, you passed the wake-up recording procedure! Your life is now moving in the right direction. But immediately you feel the control leaving you. You are now a passenger in said life.
Everything moves faster as you progress through medical school, internship, and residency. Nothing gets easier or slower. You thought you had more family time, but the time isn’t yours anymore.
Your relationships will be challenged, as will your sanity. You feel desperate to be competent. Financial pressure builds up. When your first doctor job comes along, roll at full speed. The ride gets bumpy as you navigate your first employment contract and learn a new EMR. And don’t forget to study your DEI package because there will be a test. Your employer expects you to treat more than thirty patients if you want credit forgiveness.
It’s a good thing you have professional liability insurance because you’re going to need it. Through no fault of your own, some of your patients are dissatisfied. But at the moment you are still working.
If you choose traditional private practice away from home, chances are your partners will want a piece of you. They want you to make money for them. “You better shop yourself,” they tell you. They won’t hesitate to see you in court if you step out of line. Now your career is progressing at lightning speed, kicking up dust from the dirt road you chose to take. At some point you can no longer hold yourself or see the path below you, so in an act of desperation you jump off, not knowing where you will land.
You will inevitably be thrown sideways, bruised and crushed. You can lick your wounds now. But no worry! You are still alive and many people love you. And because you’re a good person, a telemedicine job will be there to dust you off and hug you.

Celebrities like Canadian supermodel Winnie Harlow suffers from this condition. It can happen to anyone of any age.

Although there is no cure for the condition, medical advancements have made it possible to keep the disorder in check. Vitiligo treatment in India is gaining momentum around the world due to its cost-effectiveness and high success rates.
But, what is vitiligo?
Read on to know more.
Vitiligo: A Condition due to Changes in the Immune System
Vitiligo is a skin condition that occurs due to changes in the immune system. Our cells contain a skin-coloring pigment called melanin, produced by melanocytes. When the melanocytes stop producing melanin, our skin starts to lose color in the form of white patches. This condition is known as vitiligo.
It starts with the appearance of pale patches on the skin, called depigmentation, which continues to grow over time. Some may experience itching before a new patch appears. Furthermore, a person may also see the whitening of hair strands and eyebrows.
Causes Of Vitiligo
Vitiligo is classified as an autoimmune disorder. It occurs when our immune system starts destroying the melanocytes. Researchers have added that family history and genes may also play a role in causing this condition. At times, events such as exposure to a chemical, sunburn, or emotional stress may trigger vitiligo or, in some cases, make it worse.
Vitiligo may occur if there is a family history of the disease or if the person is already inflicted with an autoimmune disorder, like:
- Addison’s disease
- Pernicious anemia
- Psoriasis
- Rheumatoid arthritis
- Systemic lupus erythematosus
- Thyroid disease
- Type 1 diabetes
Complications due to Vitiligo
Vitiligo might not be a painful condition but may cause quite a few complications. This is due to exposure of bare skin to outside elements. It includes:
1. Increased likelihood of getting autoimmune disorders
People affected by vitiligo have higher chances of contracting other autoimmune diseases such as diabetes, anemia, and hypothyroidism.
2. Sunburns due to sensitive skin
Tanning is a natural reaction of our skin to sun exposure. When we get under the sun, the melanocytes in our skin start producing more melanin to protect our skin from potential sunburns. This process doesn’t take place when a person suffers from vitiligo.
The pale white patches of vitiligo are devoid of melanin which increases the sensitivity of the skin. Because of this, our skin would get burned rather than simply getting tanned.
3. Lower self-esteem
Vitiligo can cause someone to feel very insecure about their appearance. It can damage their self–confidence and how they view themselves because it causes their skin to have patches of different colors.
4. Eye abnormalities
In some people, vitiligo may affect the eye retinas which contain light-sensitive cells and create color variation in their irises. Although you may encounter inflammation in these areas, your vision won’t be affected.
Diagnosis of vitiligo
Identification of depigmented spots on your skin is the first step in diagnosing vitiligo. If the doctor confirms your suspicions or identifies the pale spots as macules (vitiligo–inflicted pale spots on the skin), they will move forward with the diagnosis by asking several questions:
- Whether you have been diagnosed with an autoimmune disorder
- Whether a close relative has been diagnosed with vitiligo
- If you have been under stress recently or went through an emotionally triggering event
Doctors may use equipment such as Wood’s lamp (a tool to identify pigment loss using UV rays) to look for depigmentation. The presence of pale patches is a common symptom of many conditions such as sunburn, chemical leukoderma, and even albinism.
So, to get a clear picture, doctors may suggest you undergo a biopsy to check the presence of melanocytes. Additionally, you might also be asked to take a blood test to check for other autoimmune disorders.
Furthermore, your doctor may also check for uveitis, an eye inflammation that can be associated with vitiligo.
Vitiligo Treatment
No cure has been developed for vitiligo to date due to the uncertainty of its origin. Scientists have yet to identify the exact cause of this autoimmune disorder. Although this skin condition cannot be healed, treatments can slow it down or stop its progress, help melanocytes grow again, and make the patchy skin look better by returning some color to white areas.
For temporary relief, you can use makeup like skin tanners to cover the skin patches. For hair, you can use hair dye to bring color back.
If you are looking for long-term relief, your doctor may suggest you some medications and therapies such as:
- Pigment removal from unaffected skin using monobenzone cream
- Vitamin D ointment/cream
- Combination therapy (PUVA) with UVA light and the oral medication Psoralen (photosensitizing drug)
- Corticosteroid creams (medicine to reduce inflammation)
- Ointments containing immunomodulators (a substance that suppresses or stimulates the immune system to fight certain diseases)
- Plastic surgery or Skin grafting
- Excimer laser (with UVB light)
These medications and therapies will help reduce the pigmentation, resulting in an even skin tone.

‘For me, ramadan is not just abstaining from food and water, it is a complete detoxification of the body and mind,” says Tahseen Akhtar, 28.
Tahseen is one of the two billion Muslims around the world who are currently observing Ramadan.
In the next month, followers of Islam will devote time to reflection, increased worship, charity, fellowship and fasting.
It’s a time of self-evaluation and improvement – a chance to strengthen connection with faith, God and community.
And for many, this disciplined routine and focus on mindfulness is a huge boost to their mental health.
Tahseen, from Manchester, is terrified and says Ramadan is helpful in managing her condition. “Since Ramadan is a time for prayer and reflection, this helps me stay calm and grounded, which allows me to adapt better coping mechanisms if any anxiety symptoms arise.
“I definitely have the feeling that my symptoms such as restlessness, nervousness and overthinking are significantly reduced during Ramadan.
“It allows me to look within and reconnect with myself. In the hustle and bustle of everyday life, we often lose sight of the essentials.”
However, due to the challenges the month can bring, she will also have to adjust some of her usual coping mechanisms.
“I find the gym to be really helpful for my mental wellbeing, but in Ramadan I can’t commit to intense workouts, so I’ll try to walk 10 minutes a day, which helps to refresh my mind and make sure I’m that I still get my body moving.’
Nubaid Haroon, 31, also says Ramadan reduces his anxiety. “For me, Ramadan is all about concentration,” he explains. “I’m fortunate to say I haven’t suffered from depression, but anxiety is a regular occurrence – during Ramadan I really focus on my inner beliefs that allow me to stay in the present moment, which we don’t can do our everyday life.
“During Ramadan you switch off from all the noise and become one with the idea of being here today and nowhere else.”
Bradford’s Nubaid also says he experiences more gratitude during the holy month. “Everything slows down during Ramadan, we appreciate everything around us and the tiny details that God has given us,” he says.
“This positive element helps to deal with emotions in everyday life. After Ramadan I feel a sense of warmth towards life and a strengthened gratitude.”
For Sebina Hussain, 31, Ramadan previously helped her cope with postpartum depression.
She says, “Last year I was struggling with postpartum depression and found that Ramadan pushed me to develop a routine that has helped me.”
New moms aren’t expected to fast during the month, but Sebina, from London, says skipping that part of the month has actually had a negative impact on her in the past. “I felt like I was missing something,” she says.
And Sebina says doing other exercises throughout the month also boosts her mental well-being.
“I find that taking time in my day-to-day routines to focus on reciting prayer is very healing.
“I also find that because others around me are also focusing on spirituality this month, the overall atmosphere is helping to improve my mental health.”
The positive effects last well beyond the holy month. “I see Ramadan as an annual detox,” she says. “It’s helping me improve my diet and time management, which in turn allows me to develop new habits that I hope to continue beyond Ramadan.
“This year, Ramadan coincides with the Easter break from school, so I’m happy to host my loved ones.”

Cognitive-behavioural psychotherapist Humza Khan shares his thoughts.
“A crucial part of self-care during Ramadan is compassion – being kind to ourselves. That means listening to your body and respecting it. Ramadan is an opportunity to start building new and healthy habits or to strengthen existing practices. We can apply aspects of self-care and mindfulness from Ramadan to our daily lives, whether establishing a new routine or structure for what we choose to do each day.
“Ramadan is an opportunity to shift our focus away from worries that we cannot control or that are not happening now by working towards being present. As we pray we always work to be as present as possible while concentrating on our attitude, attitude, the verses recited and the actions we need to take.’
